Pathways Advisory Service

We would like to introduce two new members to our team: Kate from Jobcentre Plus and Moira from NHS Condition Management Team. They are representatives of Pathways Advisory Service a joint partnership between Jobcentre Plus and The National Health Service.

We would like to encourage patients who feel they might benefit from this new service to take up the opportunity of visiting the Pathways Advisory Service Team in the surgery. Participation is completely voluntary and will not affect your entitlement to benefits. Feedback from patients who have already accessed the service has been very positive and the team have been able to assist service users make a more informed decision about their own futures.

The team will be offering advice and information on the support now available through Pathways to Work. The Pathways to Work Choices Provision offers:

Access to a Personal Adviser Service.
Access to Condition Management Practitioner (CMP) to help you acquire skills that improve understanding and management of your health conditions.
A structured programme of services and financial help, which empowers patients who have expressed an interest in returning to work to make a more informed choice about their own future.
Access to local training and development courses.
Advice regarding the Jobcentre Plus process for medical referrals.

Thursday 9.00am to 11.30am The Kelburn Practice
Friday 9.00am to 11.30am The Kelburn Practice
Friday 1.30pm to 4.00pm The Barony Practice
